How do you connect a piezo to a frequency amplifier/generator?

I need to know a how a piezo can be connected to a frequency amplifier / generator so that I can make the piezo move linearly and vary the frequency of the piezo and some readings can be taken. Please tell me how this can be done exactly.

How would a piezo that has three leads coming out of it connect to a frequency amplifier?

PS, there is no such thing as a "frequency amplifier" that I know of.
